How long does recovery after HIFU take?

Physical recovery is generally fast: most patients are discharged the same or the next day and return to daily life within days. The fibroid’s shrinkage runs on its own separate timeline: clinical series report average shrinkage of about a third at 6 weeks, half at 6 months and further by 12 months. Separating these two timelines is the key to setting expectations properly.

The recovery timeline: your body in the first weeks

    First 24–48 hours

    Discharge after brief observation; mild-to-moderate lower-abdominal pain, skin sensitivity and tiredness can occur. Simple pain relief is usually enough.

  2. 02

    First week

    Mild pain and discharge may occur and usually settle on their own. Beyond avoiding heavy lifting there are few restrictions; desk-based work is commonly resumed within this period.

  3. 03

    Weeks 2–4

    Daily life returns to normal. Do not expect visible shrinkage yet — resorption is only beginning in the background.

  4. 04

    Months 1–3

    The first follow-up imaging usually falls in this window; the first measurable shrinkage and early symptom easing are common.

  5. 05

    Months 6–12

    The main period of shrinkage; outcomes are consolidated with symptom review and imaging.

The shrinkage timeline: what do the numbers say?

Evidence
In a European USgHIFU series, mean fibroid volume decreased by about 33% at 6 weeks, 51% at 6 months and 59% at 12 months; a 500-case Asian series reported roughly 40% shrinkage at 3 months.

These are series averages; your fibroid’s course can differ with tissue characteristics and ablation extent.

Why does shrinkage take time?

HIFU does not "burn away" the fibroid; it inactivates the tissue. The inactivated tissue is then cleared by the body’s own mechanisms — immune-mediated resorption — shrinking over months. That is why the fibroid is still visible on imaging right after treatment, and that is not a sign of failure; success is judged by loss of perfusion on follow-up imaging and volume change over time.

Evidence
The strongest predictor of technical success is the non-perfused volume ratio (NPV) achieved: with sufficient ablation, shrinkage is greater and long-term retreatment likelihood lower.
