Is HIFU a risky treatment?

Because it is incision-free, HIFU has a generally low complication profile compared with surgery — but no medical procedure is free of risk. Temporary pain and skin sensitivity are relatively common; serious complications are rare. The limitation that deserves the most discussion is that treatment can fall short in a proportion of patients, who may need further treatment over time.

The possibility of insufficient ablation

The technical success of HIFU is measured by how much of the fibroid is inactivated (the non-perfused volume ratio, NPV). In some fibroids — especially those that appear bright on MRI — sufficient ablation is hard to achieve, and symptoms may not improve enough.

Pregnancy and fertility: the evidence is not settled

Data here are conflicting: a 2024 meta-analysis found lower pregnancy and live-birth rates after HIFU compared with myomectomy, while a 2023 meta-analysis reported similar live-birth rates across methods.

For patients planning pregnancy, method selection should be individual and this uncertainty discussed openly; a definitive claim that HIFU preserves fertility is not supported by current evidence.
