How does HIFU inactivate a fibroid?

HIFU combines many ultrasound waves from outside the body at a single focal point. Individually the waves pass through tissue harmlessly; only at the focus do they reach sufficient energy. There the tissue heats within seconds and its proteins denature irreversibly (coagulative necrosis). These small ablation volumes are applied side by side until the targeted part of the fibroid is inactivated.

Focusing: beyond the magnifying-glass analogy

The classic analogy holds: as a magnifying glass concentrates sunlight, a HIFU transducer concentrates ultrasound energy at a target. The critical difference is control: the focal point’s depth and position are planned to the millimetre, the applied energy is dosed, and each pulse (sonication) is verified under imaging. Outside the focus, energy density stays below the threshold of tissue damage.

What the heat does: the nature of ablation

At the focus, temperature quickly exceeds the threshold of cell death; the target undergoes coagulative necrosis — controlled inactivation of tissue. The inactivated tissue is not expelled at once; the immune system resorbs it over weeks to months. Why does real-time monitoring matter?

Throughout treatment the target region is imaged: position is confirmed, treated areas are marked, and the tissue’s response to energy is observed. Breathing movement and tissue response are accounted for this way. Image guidance is what makes HIFU a guided therapy rather than a blind application of energy.

Ultrasound-guided or MR-guided?

Two system families use the same physics with different guidance. Ultrasound-guided HIFU (USgHIFU) follows the procedure on real-time ultrasound; MR-guided focused ultrasound (MRgFUS) works inside an MRI with temperature mapping. The literature evaluates the two separately; the systems described on this site are ultrasound-guided.
